Records Requests

All requests for records must be submitted in accordance with the Government Records Access and Management Act (GRAMA). This refers to state law governing the privacy of records and conditions upon which they can be released, found in Title 63G, Chapter 2. You may visit the Division of State Archives & Records Service Website for additional information.

The State of Utah has created a new website from which record requests may be submitted to a governmental agency. To Request records from the Division of Services for People with Disabilities, please visit the State Open Records Portal. Records requests may still be submitted through the United State Postal Services. Submission by either method are processed in the same manner.

Please do not submit duplicate requests.
